- comments: observed evidence of rodent activity on the premises. Observed approximately 250 rat droppings in the folowing areas: under storage shelf and behind the the compartment sink in the bar area, rear corner of dish machine, behind bulk powders on wheels, under 2-door cooler, side of walk-in cooler, under the grease trap, under the grill/cooking area, on top of containers in the hallway, basement corner, the ledge in along the basement, table in the basement, the boiler room located in the basement. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority #7-38-020(a). Citation issued.

- comments: observed a hole outside leading inside where the air conditioner system connects. Instructed to seal the hole. Observed soiled floors underneath all cooking equipment in the kitchen. Instructed to detail clean throughout the entire kitchen.
