8. Hands clean & properly washed
- comments: observed male employee several times handle money with bare hands,and then use hand sanitizer and put on another pair of gloves and handle ready to eat bread and tcs items without washing hands. Must wash hands after every different task. Instructed employee on when to wash hands. (cos)(priority 7-38-010)(citation issued)

10. Adequate handwashing sinks properly supplied and accessible
- comments: must provide hand washing signs at hand sinks.

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 40 or more mice droppings on lower shelf in between cords and computer equipment at cashier station. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 1/29/21. (priority foundation 7-38-020(a)(citation issued)

39. Contamination prevented during food preparation, storage & display
- comments: must provide a splash guard at front hand sink next to cups and lids or remove items from area and at 1- compartment sink next to rear prep table.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must remove cardboard from walk in cooler shelving units and rear prep table. Must repair or replace damaged sink stopper handle at far right compartment. Must eliminate crates used for food storage and provide adequate shelving units.

49. Non-food/food contact surfaces clean
- comments: must clean dust build up from lower shelves at hot & cold holding units. Must clean build up on ice dispenser at soda machine.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: must provide a garbage can with lid in women's toilet room.

- comments: for purposes of this order, businesses covered by this order include any for-profit, non-profit, or educational entity, regardless of the nature of the service, the function it performs, or its corporate or entity structure. (a) in addition to the health steps required by executive order no. 2020-43, businesses must take the following additional public health measures to protect their employees, their customers, and all others who come into physical contact with their operations: (3) ensure that every employee who interacts with customers and is able to medically tolerate a mask is wearing a mask for the duration of a shift; observed male employee entering into restaurant and food prep area without face covering. Only after instructed to put a mask on. Must provide face coverings. (citation issued 7-28-060)

64. Public health nuisance
- comments: 2. In addition to the health steps required by executive order no. 2020-43, individuals must take the following additional public health steps to protect their own and their neighbors' health and lives: (b) any individual who is over age two and able to medically tolerate a mask shall be required to wear a mask when in a public place, unless otherwise provided for in the phase iv: gradually resume guidelines promulgated by the office of the mayor ('gradually resume guidelines'); observed a black male in restaurant ordering and being served several sandwichs without a face covering. Instructed manager that all customers must provide a face covering. (citation issued 7-28-060)
