25. Consumer advisory provided for raw/undercooked food
- comments: observed no consumer advisory disclosure provided on the menu for raw or undercooked foods. Management instructed to describe or disclose which foods can be ordered raw or undercooked and add a statement before the reminder statement that 'these foods can be ordered raw or undercooked. ' priority foundation violation 7-38-005.

38. Insects, rodents, & animals not present
- comments: observed approx. 40 live small flying insects on the walls, ceiling, and boxes of stock in the rear food prep area, employee washroom and storage room. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed an electric bug zapper and a fly strip mounted to the ceiling of the rear food prep area. Management instructed to remove both objects from the food prep area.

41. Wiping cloths: properly used & stored
- comments: observed wet wiping cloths stored on top of the prep tables and prep coolers. Management instructed to store wet wiping cloths in a container of sanitizer when not in use.

- comments: observed tortilla chips stored in direct contact inside a cardboard box in the food prep area. Management instructed to use food grade storage containers only.

- comments: detail clean the interior and exterior of the microwaves, storage shelves, electrical conduit behind the prep cooler, interior of prep coolers, blenders and mixers, and grease on the grill.

- comments: must reglue the loose floor tiles in the men's washroom.
