    “June 11th: I got a chicken burrito bowl with white rice, both beans, green salsa, lettuce, cheese, and sour cream. I wrapped it in a tortilla on the side. I ate at 5pm, and at 6pm had very immediate onset diarrhea. This happened around 5 more times and I was incredibly nauseous. I was hoping to slee...     - comments: observed no written procedures or supplies for responding to vomiting or diarrheal events. Supplies must include adequate sanitizer that eliminates norovirus. Instructed facility to provide clean up policy and supplies to properly handle bodily fluid spills on site. Priority foundation violation #7-38-005. No citation issued.

    - comments: observed food handler touch ready to eat food (tortilla, tomato, lettuce, and onion dressing tacos) with bare hand. Food employees may not contact exposed, ready-to-eat food with their bare hands and shall use suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment. Priority violation #7-38-010. Citation issued.

    - comments: observed food containers and boxes stored on the floor in the walk-in freezer. Instructed manager to properly store and protect food items from contamination while stored by elevating at least 6 inches from the floor on raised shelving units.

Approximately 2-4 live flies on 16 dining tables inside dining room, windows and bar open to the outside no air curtains or any other protection from vermin entering the dining and bar area. 1 live fly on inspectors visor. 7 live flies on bus cart in dining room. 1 live fly on cutting board at cook line. 5 live flies on prep table across ice machine.

    “May 18th 2:49 pm. I purchased and drank a strawberry lemonade freeze. As I drank, it tasted off, like a cleaning product. Maybe they had just cleaned the machine and didn't rinse it out properly.

I had to hurry to the toilet, to basically pee out of my butt. Around 5 am I puked up everything. I could see there was still the soft shell tortilla undigested in my vomit, even though I had eaten their food around 15 hours prior it was still undigested in my stomach. drank water, puked again, drank more water puked a third time, until it was clear.
