- comments: all bulk food containers and foods not in the original containers, including condiments and spices, must be labeled with the product common name.

- comments: rear exit screen door with open 1/2 inch gaps along the bottom threshold and open holes in the mesh screen. Must seal all openings in the mesh screening and provide a tight-fitting seal on all sides of the door.

- comments: observed approximately 100 mice droppings in the following areas: along baseboards throughout both dry food storage rooms, behind all box freezers, on lower shelving throughout the cooks line, behind tall reach-in coolers in the front cashier area, and throughout the mop room. Also observed were over 100 small drain flies throughout the dish area, three-compartment sink, and throughout the rear dry food storage room at oil storage. Recommend having pest control co. Service establishment for mice and flies throughout. Remove all droppings, and sanitize all areas. Eliminate all fly activity throughout. Priority foundation violation 7-38-020(a) citation issued.

- comments: all food handlers must wear effective beard restraints.

- comments: all stored items throughout the facility, including any needed items stored in the basement, must be elevated and organized. Shelving for storage must be elevated to leave floors accessible for cleaning and monitoring.

- comments: must not store/use raw wood for bun storage inside the steam table.

- comments: both toilets in the washroom were noted dirty. Must clean and maintain.

- comments: all unnecessary items and unused equipment throughout the basement must be removed from the premise to avoid pest harboring.

- comments: water damaged ceiling tiles throughout the storage area and the hallway box freezer. Must replace. ----floors throughout the entire facility with dirt, debris, grease, and dead mice along baseboards and under elevated refrigerators. Excessive grease and food debris under and around the three-compartment sink, sink grease trap, unused dish machine, and cooking equipment. -----pooling liquid throughout the floors of the basement. Must clean all, remove all dead mice, and remove all standing water in the basement. ----must not use cardboard as a ceiling vent cover above the unused dish machine. Or as splash guards at the deep fryer. ----excessive grease and food splatter on the walls in the following areas: at the three-compartment sink, at the kitchen chicken prep table, at the rear hallway bulk oil storage. Must clean and maintain all. ----hole in the wall of the backsplash at the three-compartment sink. Must seal.

- comments: no proof of allergen training or certificate for the city of chicago certified food manager. Must provide.
60. Previous core violation corrected
- comments: previous core violations from report #2365716 on 3-12-20 were not corrected. #41-wiping cloth for wiping the prep table, chopping board, and coolers, must be stored on a bucket with sanitizing solution in between uses. #49- must detail clean the exterior surfaces of the grill and deep fryer with grease accumulated and interior surfaces of the deep fryer door and maintain. Priority foundation violation 7-42-090 citation issued.
