- comments: observed hand washing facilities not maintained and accessible. Observed no soap at hand sink in rear meat prep area and hand sink in produce prep area with 3- bottles inside of hand sink basin while food was being packaged in same area. Soap was provided and articles removed from sink during inspection. Must monitor and maintain at all times. (priority foundation 7-38-030(c)(cos)(citation issued)

- comments: observed evidence of appx. 100 or more mice droppings on floor in fire pump room and on pallets in milk walk in cooler. Must remove droppings. Clean and sanitize areas. Got turkey and Swiss sandwich with mayonnaise Friday, Sept 27 to go. Noticed after I ate, the napkins smelled like mold. Had some gas later in afternoon, which was extremely unusual. Next day, today, a had profuse diarhhea and associated illness (weakness etc) .
