10. Adequate handwashing sinks properly supplied and accessible
- comments: observed hand washing facilities not maintained and accessible. Observed no soap at hand sink in rear meat prep area and hand sink in produce prep area with 3- bottles inside of hand sink basin while food was being packaged in same area. Soap was provided and articles removed from sink during inspection. Must monitor and maintain at all times. (priority foundation 7-38-030(c)(cos)(citation issued)

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 100 or more mice droppings on floor in fire pump room and on pallets in milk walk in cooler. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 1/14/22. (priority foundation 7-38-020(a)(citation issued)

- comments: must remove rust from lid of grease trap in meat prep area. Must eliminate crates used for food storage throughout all walk in coolers and prep areas.

- comments: must clean dust build up on all fanguard covers in all walk in coolers,wrapping stations,undersides of prep tables & overhead shelves,grinder.

- comments: must clean dust build up from walls and ceilings in all walk in coolers and walls in prep areas. Must clean mold build up on lower wall in deli prep area.
