- comments: observed no person in charge with a city of chicago certificate. Instructed to have a certified food manager with the city of chicago certificate on site at all times.
2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on site during the inspection while the temperature control for safety foods (chicken,pork,rice,beans etc. ) are prepared and served to customers. Instructed a city of chicago certified food manager must be on site at all times foods while (tcs foods) are being handled prepared and served. Priority foundation violation 7-38-012,citation issued.
25. Consumer advisory provided for raw/undercooked food
- comments: observed no consumer advisory disclosure and reminder statement on the menus. Instructed management to disclose which foods can be ordered 'raw' or under cooked and link those foods with an asterisk (*) to a reminder statement that states that eating raw or under cooked foods can increase your risk of food borne illness. Priority foundation violation 7-38-005.

- comments: instructed to properly label food containers/bins identified with common name (salt, sugar, flour etc). In basement dry storage area.
39. Contamination prevented during food preparation, storage & display
- comments: instructed to install and maintain a washable splashguard at right side of the hand sink next to the 3-compartment sink at rear prep/dishwashing area.
55. Physical facilities installed, maintained & clean
- comments: observed dirty floors behind,under,around refrigeration units with debris in basement storage area. Instructed to detail clean floors throughout.
56. Adequate ventilation & lighting; designated areas used
- comments: instructed to detail clean kitchen exhaust hood/filters to remove hanging dust/grease build-up and maintain.
57. All food employees have food handler training
- comments: observed food handler (s) with no proof of food handler training certificates. Instructed to provide and maintain.
60. Previous core violation corrected
- comments: observed previous core violation #58 (no food allergen training certificates on premises. Instructed all city of chicago certified food managers must have food allergen training certificates on premises) not corrected within 90 day time frame on the inspection report #2320852 dated for 10/31/19. Instructed person in charge that the violation shall now classified as a priority foundation item due to non previous noncompliance. Priority foundation violation 7-42-090,citation issued.
