- comments: observed no certified food managers certificate posted to view nor on duty while tcs foods are being prepared,handled and served such as (chicken,diced tomato,lettuce). Must provide and on site at all times. (priority foundation 7-38-012)(citation issued)

- comments: observed no signed employees health policies. Must provide and maintain. (priority foundation 7-38-010)

- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)

- comments: observed evidence of appx. 40 or more mice droppings on floor and ledge of hot water tank,on lower shelf of shelving unit in rear prep area. Must remove droppings. - comments: observed an appx. '1-2' gap along top and center of double entry doors. Must make doors tight fitting.
