- comments: no handwash signage at handwash sinkds. Instd to provide and maintain

- comments: variety of foods (cooked italian sausage, cooked beef, etc) not properly labeled, dated. Refrigerated, ready to eat tcs food prepared and held for more than 24 hours shall be clearly marked to indicate the day or date by which food shall be sold or discarded for a maximum of 7 days (day of preparation is counted as day 1). - comments: previous core violations from 4/5/2019 report 2282678 16 4-602. 11(b-e): **c** equipment food-contact surfaces and utensils-frequency observed black substance on plastic interior ceiling of ice chamber, and orange/brown, slimy, mold like substance on plastic cover inside lower interior of ice machine. Instructed to detail clean and sanitize ice machine. Must maintain same. 23 3-501. 17: **pf** ready-to-eat time/temperature control for safety food, date marking observed no labels on prepped tcs foods. Must label all tcs prepped foods with name and discard date. Priority foundation violation. 47 4-501. 12: **c** cutting surfaces observed heavily soiled cutting board with deep groves on rear prep table (not in use at time of inspection). Instructed must resurface or replace. Must maintain same. 47 4-101. 19: **c** nonfood-contact surfaces observed cardboard used as a liner inside hot holding unit. Instructed cardboard not permitted, must remove. All surfaces must be non-porous, smooth and easily cleanable. Observed excessive debris on bottom of 2 door freezer in rear prep. Must detail clean and sanitize. Must maintain same. 58 2-102. 13: **c** allergen training - all certified food service sanitation managers employed by a restaurant must receive or obtain training in basic allergen awareness principles within 30 days after employment and every 3 years thereafter. Training programs must be accredited by the american national standards institute or another reputable accreditation agency under the astm international e2659-09 (standard practice for certificate programs). Observed no proof of allergen training on site. Must provide proof of allergen training for all chicago food sanitation managers. Priority foundation violation 7-42-090
