2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on site during the inspection, while the time/temperature control for safety foods was prepared on site. A certified food manager must be on site with the original city of chicago food service manager certificate at all times while foods are being prepared and served. Priority foundation violation 7-38-012, citation issued

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no handwashing signage at handwashing sink in the rear food prep/dishwashing area. Instructed to provide and maintain.

- comments: observed no handwashing sink in front of service/food prep area. Hot holding unit with meat, cheese, and a jar of peppers (for nachos) observed in the front service area where money and food are handled. Facility must obtain and install a hand sink under city water pressure and maintain it. Priority foundation violation. 7-38-030(c), citation issued

23. Proper date marking and disposition
- comments: observed improper disposition date for ready to eat tcs food items (turkey and beef sandwiches) prepared on site. Sandwiches observed with dates such as 06/30, 08/10, and 08/30 stored for sale in display cooler. No logbook was provided to see the initial date of preparation for sandwiches. Instructed to provide proper date marking with disposition date for rte foods prepared on site and held over 24 hours. Discard date must start from the first day of preparation and discarded or sold within seven days. Approximately 20lbs of at $85 worth of product discarded. Priority violation 7-38-005, citation issued

- comments: observed no ambient air thermometer in the prep cooler in the prep. Dishwashing area. Instructed to provide and maintain.

- comments: observed deli sandwiches prepared and packaged on site with no food label on display for sale. Instructed to provide food labels must contain: the name and place of business of the manufacturer, packer, or distributor, ingredients including a declaration of artificial coloring, flavoring, and chemical preserves.

46. Gloves used properly
- comments: observed boxes of examination gloves in food prep areas. Instructed not to use examination gloves to hand food, must use food grade gloves while handling food items.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility missing two sink stoppers for the three compartment sink. Isntructed to provide and maintain.

49. Non-food/food contact surfaces clean
- comments: observed the interior of the prep cooler wit slight food debris. Instructed to clean and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed the floors under the three compartment sink in need of cleaning and the floors behind the shelves where chips are stored in the shopping asiles with a build up of chips. Instructed to clean and maintain mentioned floors.
