- comments: observed person in charge without proof of food safety management training during time of inspection. Must provide.

2. City of chicago food service sanitation certificate
- comments: observed no certified city of chicago food service sanitation manager on site at start time of inspection while eggs were being cooked. The designated person in charge, who is physically on site, must obtain and maintain a valid city of chicago food service sanitation license. Priority foundation violation 7-38-012. Citation issued

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the following sinks not maintained with hot water: handsink in prep area with no hot water. Instructed person in charge hot water at all hand washing sinks must be at least 100f. Pic came on site and hot water was restored handsink are maintained between priority violation 7-38-030(c). Citation issued.

- comments: observed no handwashing signage at the bar handsink. A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees. Instructed to provide.

- comments: noted low temperature dish washing machine in use not properly sanitizing. Sanitizing strength of sanitizer (chlorine) at the time of inspection was 0 ppm. Instructed not to use unit until reinspected by chicago department of health (cdph). Tagged unit. Priority violation 7-38-025. Citation issued.

- comments: observed time/temperature control for safety foods (variety of pork sausage patty at 55. 4f) at improper cold holding temperatures located in reach in cooler. Instructed to discard and denature all said tcs foods and to maintain a cold holding temperature of 41f at all times. Priority violation. 7-38-005. Citation issued.

- comments: observed residential roach insecticide, boric acid stored in dry storage area. Informed that residential use insecticides are not permitted in commercial spaces. Priority violation. 7-38-005. Citation issued

- comments: observed facility not meeting compliance with variance/specialized process; facility curing bacon, etc in prep kitchen area. Informed person in charge of requirements and facility must meet and maintain. Priority violation 7-38-005 citation issued.

- comments: found no variance on-site for the curing process. Instructed manager to provide. Priority foundation violation. Citation consolidated with above violation 29.

- comments: observed food storage containers without labels containing the common name of foods. Instructed to provide.

- comments: observed over 300 dead small black flies through out storage on food containers, floor and paper goods. Instructed to remove all dead pests and clean and sanitize

- comments: observed cockroach infestation. Over 50 live roaches observed in the following areas: dishwash and food prep areas, in compressor of reach in cooler, on floor, containers, walls etc. 7-38-020(a) priority violation citation issued. Must eliminate all pest activity, wash and sanitize surfaces. Have premises serviced by pest control operator.

- comments: observed sewage lines over single-use items in the upstairs dry storage not properly shielded to prevent contamination. Management instructed to properly shield the sewage lines to intercept potential drips or remove the items stored beneath the sewage lines

- comments: observed milk crates being used as shelving throughout the storage areas. Must remove and provide shelving that is 6" off the floor and provides floor accessibility to ensure adequate facility cleaning.

- comments: observed facility without (chlorine and quats) chemical test kit for sanitizer or for ware washing area. Informed person in charge a test kit that accurately measures the concentration in mg/l of sanitizing solution must be provided. Priority foundation violation 7-38-005. Citation issued.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed water backing up at the 3 compartment sink area. When water is released from 2nd sink basin, water backs up into 1st and 3rd sink basins. Instructed manager to repair before re-inspection date. Priority foundation violation 7-38-025. Citation issued.

- comments: observed grease buildup on cooking equipment, walls, surrounding floors and shelving units in the kitchen/prep areas. Instructed to degrease, clean and sanitize. Including interiors and undersides of equipment and shelves/tables.

- comments: observed 3 compartment sink in dish area leaking from drainpipes, instructed to repair and maintain

- comments: observed condensate drainage water line from walk-in cooler in prep area and from 2nd floor storage area discharging into plastic bucket. Instructed to drain condensate drainage water from point of discharge to disposal, such as to a floor drain or other floor-mounted plumbing fixture, according to the law and maintain.

- comments: observed toilet room doors to not be self closing. Instructed to install a self closing device and maintain at all times

- comments: noted no access to toilet room on 2nd floor storage area, instructed to clear area and provide access to said area

- comments: observed grease buildup, food debris, and dirt on floor under cooking equipment, along walls and in all corners in prep, dish washing, storage areas. Instructed manager to clean floors under all equipment, along walls and in all corners in all areas. Observed dust buildup on fan in walk-in cooler, light shields in cooking area and ceiling and wall vents in prep and storage areas. Instructed manager to clean and maintain all fans, light shields and ceiling/wall vents.

- comments: observed a hole in the wall under 3 compartment sink and near rear walk-in cooler, and in 2nd floor storage area noted facility wall bases peeling off coving falling off instructed to repair and seal.

- comments: observed missing and damaged floor tiles throughout entire dry storage room, stairs leading to dry storage. Instructed to repair to create a smooth and easily cleanable surface. Must maintain at all times.

- comments: observed the ventilation hood and filters above the cooking equipment in need of cleaning. Instructed to detail clean and maintain at all times.

- comments: noted continued non-compliance from report 2626271 conducted on 10/30/25. Previous core violations not corrected: 49--601. 11(c): (c) nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Observed excessive amount of ice build up inside chest freezers located on the 2nd floor food storage area. Must remove ice and clean interior surfaces of chest freezers. 49-4-602. 13: nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Must not use tinfoil as a liner for food storage shelving in rear kitchen food prep area. Must be smooth cleanable surfaces 55- 6-201. 11: floors, walls, and ceilings- cleanability observed a few damaged cracked and missing floor tiles in rear kitchen, stairwells and 2nd floor food storage area. Also observed a few missing wall tiles in front kitchen area next to cooking equipment. Must repair/replace and maintain. Must be smooth cleanable surfaces. Observed peeling paint on wall behind dishwashing machine. Must repaint and maintain. 55- 6-501. 114: maintaining premises, unnecessary items and litter observed clutter and unnecessary items on 2nd floor food storage area. Must remove clutter clean and organize area due to potential pest harborage and elevate stock items off the floor at least 6 inches and maintain. Observed unused 2-door cooler next to low temperature dish machine. Must repair or remove cooler from premises. Must correct core violations by the date on the report. Priority foundation violation 7-42-090. Citation issued.
