- comments: observed no hand soap provided at the exposed hand washing sink in the middle food prep area. Management instructed to provide hand soap at all hand washing sinks at all times. Priority foundation 7-38-030(c). Citation issued.

- comments: observed no paper towels provided at the exposed hand washing sink in the rear food prep area next to the 3 compartment sink. Management instructed to provide paper towels at all hand washing sinks at all times. Priority foundation violation 7-38-030(c). See violation #10 above for citation.

- comments: no consumer advisory disclosure provided on the menu for raw or undercooked foods. Management instructed to disclose or describe the items (raw fish) that can be ordered raw or undercooked or include a statement that 'these items can be ordered raw or undercooked. ' priority foundation 7-38-005.
38. Insects, rodents, & animals not present
- comments: observed a rodent infestation throughout the premises. Over 300 mouse droppings were observed in the following areas: rear washroom on the floor between the vanity and wall, on the floor beneath the wire storage racks in the storage area outside of the washrooms, on the floor of the utility closet, on the floor of the dining area and underneath the music stand area, on the floor and on the wood supports of the beverage station, on the floor and along the walls behind the cooking equipment and prep cooler and freezer in the middle food prep area, on the floor underneath the storage shelves, behind and around the ice machine and under the dish washing machine in the rear food prep area. Management instructed to clean and remove all rodent droppings throughout the premises. Additional pest control service is needed to eliminate the pest activity. Priority violation 7-38-020(a). Citation issued.

- comments: observed wet wiping cloths stored on top of the prep tables and cutting boards in the food prep areas. Management instructed to store wet wiping cloths in a container of sanitizer when not in use.

- comments: observed single use take out containers not stored inverted at the sushi prep area. Management instructed to invert all containers to protect against contamination.

- comments: observed no irreversible registering temperature indicators on the premises to measure the utensil surface temperature inside of the high temperature dish machine. Management instructed to provide. Priority foundation violation 7-38-005. Citation issued.

- comments: clean the prep tables, wok station, deep fryer, grease traps, interior of prep coolers in the food prep areas.
