- comments: observed the following tcs foods at improper cold holding temperatures in walk in cooler : large, deep containers of barbacoa 60. 9f, pork 48. 5f , beef 50. 1f instructed to discard and denature all said tcs foods and to maintain a cold holding temperature of 41f at all times. Priority violation. 7-38-005. Citation issued.

25. Consumer advisory provided for raw/undercooked food
- comments: observed no disclosure and reminder on the menu for all animal food products that can be ordered raw or undercooked. Instructed to provide a disclosure by asterick on the menu for all items (beef burgers, eggs any style, steak) that can be ordered undercooked or raw and add a footnote reminder statement on the menu that reads *these items can be ordered undercooked or raw. Instructed to discard and denature said tcs foods and always maintain all cold holding tcs foods at or below 41f. Priority violation 7-38-005. Citation issued. Food discarded.

- comments: observed improper ambient temperature inside of the prep cooler at 53f. Ambient temperature must be at 41. 0f or below inside all cold holding units. Priority violation. 7-38-005. Citation issued. Cooler tagged and may not be used until reinspected by cdph.

- comments: observed over 125 live fruit flies throughout establishment. Found approximately 75 in the 2nd floor kitchen, 25 in the rear hallway leading to the dumpster, 10 in the 1st floor mens washroom, 5 in the 1st floor womens washroom, and 10 in the 2nd floor bar. Must detail clean and remove all evidence of pest activity on premises, and contact pest control for additional service. Priority foundation violation. 7-38-020(a). Citation issued.

- comments: observed the third handwashing sink (out of 4) from the left in the mens washroom with hot water at 88f. Must provide hot water at 100-120f at all handwashing sinks.

nationwide. There have been no confirmed reports of adverse reactions due to the consumption of this product.

The recalled product is:
- 10.7-oz. vacuum-sealed packages containing two pieces of "100% Grass Fed & Finished Beef Burger Patties 85% Lean/15% Fat" and with "Use/Freeze By 4/11/23" on the package label.

The affected product was produced on March 14, 2023, and bears the establishment number "Est. 6987" inside the USDA mark of inspection.

The problem was discovered after the establishment received multiple consumer complaints reporting consumers found white "rubber-like" material in the ground beef patty products during preparation.

FSIS is concerned that some product may be in consumers' refrigerators or freezers. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.
