These products were shipped to retail and food... See More service locations in Alaska.

The following products are subject to the public health alert:
- 12-oz. plastic container of "Charlie's DELIVERED FRESH DAILY SOUTHWEST SALAD" with best by dates 3/15/22 through 3/24/22, that may include Thai-style salad dressing, which contains undeclared peanut and wheat.
- 12-oz. plastic container of "Charlie's DELIVERED FRESH DAILY CHICKEN BLT SALAD" with best by dates 3/15/22 through 3/24/22, that may include Thai-style salad dressing, which contains undeclared peanut.
- 11-oz. plastic container of "Signature Café THAI Style Salad with White Meat Chicken" with best by dates 3/15/22 through 3/24/22, that may include BBQ Ranch salad dressing, which contains undeclared egg and milk.

These items were produced March 8, 2022 to March 17, 2022. There have been no confirmed reports of adverse reactions due to consumption of these products.

FSIS has verified the affect products are no longer available for sale but is concerned that some products may be in consumers' refrigerators. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.

The recalled Signature Farms Garden Salad was sold in 12-ounce bags in the Produce section. The recalled products have BEST IF USED BY dates of May 16 through July 4 (5-16-20 through 7-04-20). The BEST IF USED BY date is printed on the top right corner of the front of the package. The product has a UPC code of 21130 98135, which can be found on the back of the package and bears the unique plant number S5417.

Consumers are asked to check their refrigerators for the recalled product. Consumers who have purchased the product are asked to discard or return it to the place of purchase for a refund. Customers who have questions about the recall can contact Jewel-Osco at 1-877-723-3929.

Cyclospora cayetanensis is a microscopic parasite of humans. This parasite, when it contaminates food or water and is then ingested, can cause an intestinal illness called cyclosporiasis. Most people infected with Cyclospora develop diarrhea. Other common symptoms include loss of appetite, weight loss, stomach cramps/pain, bloating, increased gas, nausea, and fatigue. Vomiting, body aches, headache, fever, and other flu-like symptoms may be noted. Some people who are infected with Cyclospora do not have any symptoms. If not treated, the illness may last from a few days to a month or longer. Symptoms may seem to go away and then return one or more times.

The potential for contamination was brought to Hy-Vee’s attention when Fresh Express – which manufactures the product – announced that the U.S. Food and Drug Administration (FDA) and the Centers for Disease Control Prevention (CDC) have been investigating an outbreak of Cyclospora in the upper Midwest section of the United States. In their investigation, they have identified certain products linked to the outbreak – including Fresh Express’ private label products at retailers across the country that were impacted – including one for Hy-Vee.

The recall is limited to 12 oz. Hy-Vee Bagged Garden Salad, all UPCs, and expirations dates. The product was distributed to Hy-Vee grocery stores across its eight-state region of Iowa, Illinois, Missouri, Kansas, Nebraska, South Dakota, Minnesota, and Wisconsin. No other Hy-Vee food items are impacted by this recall. Hy-Vee has not received any reports of illness due to consumption of its Hy-Vee Bagged Garden Salad.

Customers who purchased this product should discard it or return it to their local Hy-Vee store for a full refund. Symptoms of cyclosporiasis begin an average of seven days after ingestion of Cyclospora. Symptoms of cyclosporiasis may include: watery diarrhea (most common), loss of appetite, weight loss, cramping, bloating, increased gas, nausea, fatigue, vomiting, and low-grade fever. Husband became violently ill <2hrs throughout evening & thru most of morning. Worst of symptoms subsided after having him drink small sips of bottled plain water, lime Gatorade, diluted Perrier water, NO solid foods. All of these cases were new packages, not the same one. So after googling about my symptoms I realized how many other people suffer the same by eating this burger. They all reported the same symptoms and apparently is a food poisoning based on a particular ingredient they use to add beef flavor/color to the burger. That night I fell ill with diarrhea and was in and out of the bathroom most of the night. The following morning I was nauseated and began vomiting. The diarrhea was still present, as was the pain. I took Emetrol and Pepto Bismol, but no relief. The symptoms continued and I saw my doctor on Friday, May 6th. I was prescribed Azithromyacin and told to continue the BRAT diet and to stay hydrated. I took the 3 day course of antibiotics and it seemed to work. The nausea and diarrhea subsided on Monday May 9th. I went back to my doctor for an all clear to return to work. To this day I am still experiencing pain in my liver and light colored stools.
