- comments: found no valid city of chicago food service sanitation manager certificate posted on site while time/temperature control for safety foods (chicken) are stored. A valid city of chicago food service sanitation manager
certificate must be on site at all times while tcs foods are stored, prepared, and served to the public. Priority foundation violation. 7-38-012. Citation issued.

- comments: observed no employee health policy on site. Instructed to provide an employee health policy that includes a signed acknowledgement from each employee. Priority foundation. 7. 38. 010.

- comments: observed no vomit and diarrhea procedure or cleaning materials to clean up said incident on site. Facility must provide procedures and cleaning materials (gloves, mask, disposable mop, disinfectant rated for norovirus) for employees to follow and use when responding to vomiting or diarrheal events. Instructed manager to provide. Priority foundation violation. 7-38-005.

- comments: found no hot water at handwashing sinks located at front bar, also noted no hotwater at handwashing sink in basemant bar, basement women and mens washroom. Must provide a minnimum hot wtaer temperature of 100f at all times. Priority violation. 7-38-030(c). Citation issued.

- comments: found no handwashing signage at bar handsinks. Instructed manager to provide.

- comments: found a minimal amount of black substance inside panel of ice machine. Must clean sanitize and maintain.

- comments: observed ceviche and burgers on menu said items can be ordered raw or under cooked. Instructed facility to provide footnote, disclosure and reminder for menu items that can be ordered under cooked. Priority foundation violatio. 7-38-005.

- comments: found sugar, flour and black beans not in its original container. Must label food storage containers when food is not in the original container.

- comments: found approx 10- 15 small flies throughout front bar. Must minimize small fly activity. Recommended pest control. Priority foundation violation. 7-38-020(a). Citation issued.

- comments: found storage area(coolers, dry goods, liquor, beverages, cleaning materials) floors, walls, and ceiling not smooth and easily cleanable. Storage area walls, floors and ceilings are brick, wood and concreate. Instructed manager to provide non-poreous smooth and easily cleanable floors walls and ceilings. You can only gain acess to storage area outdoors in rear.

- comments: found walls behind fryer filled with grease. Instructed manager to clean sanitize and maintain.

- comments: found no proof of allergen training on site at time of inspection. Instructed to provide allegren training for all city of chicago food service managers.
