- comments: no city of chicago certified manager on duty while handling tcs food (nachos with ground beef and cheese). Instd to have on duty at all times. Citation issued priority foundation 7-38-012
10. Adequate handwashing sinks properly supplied and accessible
- comments: handwash sink in toilet room and in rear prep/warewashing area not accessible, boxes on top of sink and stock in front of basin. Instd to maintain clear and accessible at all times. Priority foundation 7-38-030(c) citation issued.

- comments: over 30 mice droppings scattered on floor under display shelves and on storage shelves at front counter. Instd to remove all droppings, clean and sanitize all affected areas and have updated pest control service. Extreme clutter (boxes, etc)/stock on floor through out facility providing harborage conditions. Instd to organize premises, elevate all stock and remove clutter to eliminate harborage conditions. Citation issued 7-38-020(a) priority foundation
39. Contamination prevented during food preparation, storage & display
- comments: no barrier between customers and rear food prep/warewashing area area is located in rear where display shelves located and customers have full access. Instd to use proper barrier (wall, door, etc) to prevent possible contamination. Citation issued priority foundation 7-38-005.

- comments: employee missing food handler training. Instd to obtain training and maintain verification on site.
