- comments: interior of the upper portion of the ice machine where ice for consumption is dispensed with a black and pink slime substance. Instructed to discard all ice, clean and sanitize all effected areas. Priority foundation violation 7-38-005 citation issued.

- comments: missing internal thermometer inside the one door cooler with liquid egg product. Must replace.

- comments: observed approximatly 25 to 30 mice droppings along the basboard behind the hot water tank in the rear storage area. Recommend having the pest control co. Service for mice. Must remove all droppings, sanitize all areas. Priority foundation violation 7-38-020(a) citation issued.

- comments: observed open 1' gap with a torn/missing rodent proof strip along the bottom threshold exit door at the rear storage area. Instructed to provide a tiht fitting seal on the bottom of the door.
