- comments: observed no person in charge with a city of chicago certificate. Instructed to have a certified food manager with the city of chicago certificate on site at all times.

2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food service manager on site and no city of chicago certifed food service manager certificate posted on site. Instructed facility that a city of chicago certified food srvice manager must be on site at all times when (tcs foods) are prepared,handled,and/or served. Instructed to comply and post original certificates as required. Priority foundation violation 7-38-012.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no written employee health policy on the premises. Management instructed to provide a signed employee health policy for each employee. Priority foundation violation 7-38-010.

5. Procedures for responding to vomiting and diarrheal events
- comments: found no procedure/plan for responding to vomiting and diarrheal events. Instructed to provide and maintain required procedure/plan and appropriate supplies or kit. (necessary items at a minimum: gloves, face masks, disposable mops and appropriate sanitizer/spray that its effective against norovirus). Priority foundation violation 7-38-005.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hand sink in the kitchen prep area. Instructed to install handsink with hot and cold running water, equipped with soap and paper towels in the kitchen prep area. Priority foundation violation 7-38-030(c).

- comments: observed no metal stem thermometer for taking food temperatures. Instructed to provide and maintain.

- comments: instructed to properly label food containers/bins stored in kitchen dry storage area must be identified with common name (salt, sugar, flour etc. )

- comments: observed evidence of pest activity approx: 20 rat droppings on the behind under and around the furnace areas located adjacent to the dining room floor and approximately 10 rat droppings in a cluttered storage room adjacent to the kitchen also, observed approxmately (11) live cockroaches in interiors of the lower kitchen cabinets in kitchen prep area. Instructed to detail clean and remove all rodent droppings and cockroaches and sanitize all affected areas in detail,additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a).

- comments: observed no pest control log book provided on the premises. Instructed to provide a pest control log book from a licensed idph pest control company with required documents. Priority foundation violation 7-38-020(c).

- comments: instructed to detail clean the interiors of upper and lower kitchen cabinets with spilled food debris, food storage containers, interior and exterior of the freezer and walk-in-cooler, stove/oven grill and deep fryer.

- comments: instructed to detail clean and organize storage room located adjacebnt to the kitchen prep area and remove unnecessary articles/broken kitchen equipments due to potential pest harborage and elevate mops,brooms cleaning equipments off the floor at least 6' inches to facilitate cleaning and maintain.

- comments: observed a gas connected cooking stove in employee pantry. Instructed to provide an exhaust hood or completely remove cooking stove.
