Violations:
The inspector noted employees not washing hands properly is a big problem at Cottage Express, also there is a roach infestation in Cottage Express with roaches contaminating food.

Employees were washing hands for less than 5 seconds with no soap during the inspection.

Imitation crab, pork, chicken, and shrimp were being cold held at unsafe temperatures above 41F and had to be thrown away.

The meat grinder had visible raw meat residue on the food contact portions of it, and a roach crawled out of the meat grinder. Several live roaches were in the potato starch and flour bulk bins.

Another live roach was observed on the prep table. Large amounts of flies were observed during the inspection as well.

A dead roach was even seen on the cutting board that an employee was actively using. Dozens of dead roaches were throughout the Cottage Express chinese restaurant stuck on walls, stuck on dry storage bins, and in cabinets.
