- comments: observed no written employee health policy on the premises. Management instructed to provide a signed employee health policy for each employee. Priority foundation 7-38-010. No citation issued.

- comments: observed food provided from an unapproved source being sold on the premises. Management did not have an invoice for the prepackaged bread and there was no label on the product indicating where it was produced. Management voluntarily discarded all bread from an unapproved source. Priority 7-38-005. Citation issued.
21. Proper hot holding temperatures
- comments: observed improper hot holding temperatures of tcs foods inside of the hot holding cabinet. Observed pork at 56. 7f, lamb at 57. 9f, and stuffed peppers and cabbages at 83. 1-84. 2f. Management voluntarily discarded approximately 30# of food worth $200. Priority 7-38-005. Citation issued.
29. Compliance with variance/specialized process/haccp
- comments: observed no haccp plan on the premises for smoking or vacuum packaging meats and vegetables on the premises. Observed smoked meat (bacon, sausage, beef, etc) stored in vacuum packaging in the deli case and walk-in freezer and walk-in cooler. Observed a working vacuum packaging machine in the basement. Premises smokes meat for preservation and vacuum packages it without an approved haccp plan. Approximately 1030# of vacuum packaged meat and pickled cabbages were tagged held for inspection and placed in separate containers. The vacuum packaging machine was also tagged held for inspection. Management instructed not to vacuum package meat or vegetables and smoke meat without a city of chicago approved haccp plan.
32. Variance obtained for specialized processing methods
- comments: no variance or haccp plan provided for smoking and vacuum packaging meats and vegetables on the premises. Management instructed to have an approved haccp plan and variance approved by the city of chicago. Priority foundation 7-38-005. No citation issued.
