3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: found no employee health policy on site. Instructed managers to provide a verifiable acknowledgement of their responsibility to report information about their health and activities as they relate to diseases that are transmissible through food to the pic. Priority foundation violation. 7-38-010. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: found no handwashing signage located in mens toilet room. Instructed manager to provide handwashing signage to all handwashing sinks, must maintain.

36. Thermometers provided & accurate
- comments:. Found no thermometers inside any coolers on site. In a mechanically refrigerated or hot food storage unit, the sensor of a temperature measuring device shall be located to measure the air temperature or a simulated product temperature in the warmest part of a mechanically refrigerated unit and the coolest part of a hot food storage unit. Instructed to provide.

37. Food properly labeled; original container
- comments: found no labels on bulk food items taken out of their original container. Instructed manager to provide common name to all food items taken out of original container.

38. Insects, rodents, & animals not present
- comments: found approx. 50 rat droppings under the staircase next to the walk-in freezer. Also noted are approx 50 mouse droppings under all shelves in the front preparation area and the cabinet where cups are stored. Recommended contacting pest control. Found no live pest activity at the time of inspection. Must eliminate pest activity and remove all mouse droppings. Instructed to clean, sanitize and maintain said areas. Priority violation. 7-38-020(a). Citation issued.

- comments: found a variety of meat stored on floor in walkin cooler. Instructed manager to elevate ll food items 6 inches off of floor.

- comments: found gasket on freezer located in middle hallway torn and hanging. Instructed manager to repair and maintain.

- comments: found interior of cooler filled with an excessive amount of black mold like substance. Noted shelves in cooler filled with unknown substance. Instructed manager to clean sanitize and maintain.

- comments: unable to locate the backflow prevention device on utility sink. Instructed to locate.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: found uncovered garbage receptacle located in womens toilet room. Must provide a covered garbage receptacle to any toilet room women utilize.

- comments: found floor tile missing in rear prep. Instructed manager to replace and maintain, smooth and easily cleanable floors.

Also observed an excessive amount of water under handwashing sink in stirfry prep area and under cooking equipment in bkery prep are. Instructed to clean, sanitize and maintain.

- comments: found hole in wall under staircase directly next to walk in freezer. Instructed manager to repair and maintain to prevent pest harborage.

- comments: found food handlers without proof of food handler training. Instructed manager to provide proof of food handler training for all food handlers.

- comments: found city of chicago food service manager without proof of allergen training on site at time of inspection. Instructed to provide proof of allergen training for all city of chicago food service managers.
