Report date: Jun 10, 20262. City of chicago food service sanitation certificate
- comments: no city of chicago certified food manager on site or valid documentation on the premises while tcs foods (rice, beef, chicken, soups etc. ) is being handled and prepared. Instructed the person-in-charge that a city of chicago certified food service manager with an original city of chicago sanitation certificate must be on the premises at all times when (tcs foods) are prepared, handled, and/or served. Priority foundation violation 7-38-012, citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed handwashing sink not accessible for use while employees actively preparing foods in the bakery prep area at this time. Hand sink being blocked by cleaning equipments-mop bucket, vacuum cleaner also, storing tools/electrical extension cords inside handsink. Instructed manager, hand washing sinks must be accessible for employees to wash hands at all times. Priority foundation violation 7-38-030, citation issued.
21. Proper hot holding temperatures
- comments: observed improper hot holding temperatures of (tcs foods) held in the rear hot holding rolling unit. Observed chile cheese rellenos at 122. 8f and chicken soup at 116. 2f and chicken tenders at 106. 8f. Instructed manager, tcs foods must maintain cold holding temperature of 41. 0f or colder and hot holding temperature of 135f or hotter at all times. Management voluntarily discarded approximately 15#'s of food worth $200. 00. Priority violation 7-38-005, citation issued.
38. Insects, rodents, & animals not present
- comments: observed gap opening approx 1/2"inch at bottom right corner of the rear exit door thta leads to the outside garbage dumpster. Instructed to make door tight fitting to prevent pest entry and to maintain.
40. Personal cleanliness
- comments: observed employee with no hair restraint in the tortilleria food prep area. Instructed manager to provide hair restraints for all employees in the food prep area.
41. Wiping cloths: properly used & stored
- comments: all prep area wiping cloths must be held in a container with a sanitizing solution between use.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed stained cutting boards in the kitchen/bakery prep areas. Instructed to resurface or replace the cutting boards and maintain.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed manual sink stoppers loose/in poor repair for the rear main kitchen 3-compartment sink area. Instructed to repair and maintain 3 adequate working sink stoppers.
49. Non-food/food contact surfaces clean
- comments: instructed to clean the following equipment and maintain: prep tables (tops and bottoms) including legs of equipments, interior surfaces of fryers and other cooking equipments, under grills and shelves inside of refrigeration units, rolling baking carts.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacles provided in the women washroom. Must provide and maintain.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside commercial waste grease container not maintained with grease on top and sides of said container dripping onto ground floor. Instructed to detail and clean and maintain. Priority foundation violation 7-38-020(b), citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed food debris and spills on the floors and surfaces throughout the rear main kitchen/bakery and in the rear produce. Instructed to detail clean under all shelving units, around dish machine, prep tables and all cooking equipment to remove spills, food debris, and dirt buildup. Instructed to detail clean and maintain at all times.
55. Physical facilities installed, maintained & clean
- comments: all unused equipment materials stored piled in the rear compressor room must be removed from the premises and all other need items must be elevated from the floors.
56. Adequate ventilation & lighting; designated areas used
- comments: instructed to detail clean the rear main kitchen exhaust hood/filters to remove accumulation of grease build-up and maintain.
