Report date: Apr 23, 20215. Procedures for responding to vomiting and diarrheal events
- comments: procedure/plan for responding to vomiting and diarrheal events on site but has in-complete clean-up items. Instructed to provide and maintain required procedure/plan and appropriate supplies or kit. (necessary items at a minimum: gloves, face masks, disposable mops, and appropriate sanitizer/spray that is effective against norovirus). Priority foundation violation 7-38-005.

- comments: observed (tcs foods) at improper temperatures inside walk-in-cooler. Observed meat balls at (44. 7 f-46. 8 f),cooked eggplant at (44. 6 f-49. 7 f),calamari at (45. 3 f-49. 8),octopus at 44. 4 f,squid at 43. 8 f,sausage at 48. 0 f. Instructed manager, (tcs foods) must maintain cold holding temperature of 41. 0f or colder at all times. Priority violation 7-38-005, citation issued. (cos).
25. Consumer advisory provided for raw/undercooked food
- comments: found no consumer advisory disclosure and reminder statement on the menus. Management instructed to disclose which foods can be ordered 'raw' or under cooked and link those foods with an asterisk (*) to a reminder statement that states that eating raw or under cooked foods can increase your risk of food borne illness. Priority foundation violation 7-38-005.

- comments: observed no splashguards at the following handwashing sinks at front pizza prep handsink next to prep table and at bar handsink next to ice bin. Instructed to install washable splashguards and maintain.

- comments: observed employees with no hair restraints in the kitchen food prep area. Instructed manager to provided hair restraints for all employees in the food prep areas.

- comments: observed stained cutting boards in the kitchen food prep areas. Instructed management to resurface or replace the cutting boards and maintain.

- comments: observed ice build up inside the rear 2 door stand-up freezer on shelves. Instructed to remove ice build-up and maintain.

- comments: observed water leak drain pipe underneath the middle compartment of the kitchen 3-compartment sink. Insrtucted to repair plumbing leak and maintain.

- comments: observed rear unisex employee washroom door not self closing. Instructed to install self closing device and maintain.

- comments: observed water stained ceiling tiles and some missing ceiling tiles at rear walk-in-cooler/dry storage/ice machine areas. Instructed to replace water stained ceiling tiles and provide missing ceiling tiles and maintain.

- comments: instructed to detail clean and organize rear utility mop sink room and remove unnecessary articles due to potential pest harborage and elevate mops,brooms cleaning equipments off the floor at least 6' inches to facilitate cleaning and (no milk crates for storing of items).

- comments: observed large opening on wall at rear across from the ice machine. Instructed to seal opening and maintain.

- comments: observed missing light bulb shield for light bulb inside the walk-in-cooler. Instructed management to provide light shield or shatter-proof light bulb.

- comments: instructed to detail clean kitchen exhaust hood/filters to remove accumulations of dusts grease build-up and remove cardboard liners on exhaust hood.
