- comments: premises has no valid city of chicago certified manager on duty as required.
2. City of chicago food service sanitation certificate
- comments: no valid city of chicago certified manager on duty/site while tcs foods are being prepared, handled and serverd such as ground beef and cheese sauce for chips and italian beef. Instructed manager that a certified manager must be on duty at all time while tcs foods are available. Priority foundation citation issued #7-38-012.
5. Procedures for responding to vomiting and diarrheal events
- comments: premises has no procedure or supplies for responding to vomiting and/or diarrhea events as required. Must provide and maintain. Priority foundation violation #7-38-005.
38. Insects, rodents, & animals not present
- comments: observed over 50 mice droppings scattered on the floor around hot water tank and inside the wall. Must remove and detail clean affected area. Priority foundation citation issued #7-38-020.

- comments: must detail clean all display storage shelving,counter tops, prep stations, interior of all coolers and freezer and inside storage cabinets to remove spills and debris.

- comments: observed no running water at any of the sinks inside facility as required. Instructed to provide and maintian. Priority citation issued #7-38-030(c).

- comments: must detail clean all fixtures inside the washroom.

- comments: observed missing and damaged ceiling tiles through out premises. Must replace and maintain.
