- comments: observed no person in charge with valid chicago food sanitation certificate. Instructed manager to designate person in charge during all hours of operation. Priority foundation

2. City of chicago food service sanitation certificate
- comments: observed no chicago food sanitation manager on site or valid certificate posted when tcs food (beef 199. 3f) prepared and served. A certified manager must be on site and valid certificate posted at all times tcs food prepared and served. Priority foundation 7-38-012 citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: premises has no clean up procedure or supplies for responding to vomiting and/or diarrheal events. Instructed manager to provide. Priority foundation violation #7-38-005 citation issued.

- comments: observed no hand washing signs at hand washing sinks in prep and washroom areas. Instructed manager to provide.

- comments: observed chicken wings stored at improper temperature 109. 2f on prep table. Instructed manager to discard. All hot food must maintain 135f or above during storage, display, prepartion, and service. Priority 7-38-005 refer to previous violation for citation

- comments: observed pizza puff stored at improper temperature 58. 8f in prep cooler. Instructed manager to discard. All cold food must maintain 41f or below. Priority 7-38-005 citation issued

- comments: observed prep cooler not maintaining proper temperture 62. 5f instructed manager to repair and maintain 41f or below. Priority 7-38-005 citation issued. Cooler is tagged held for inspection and must not be used until reinspected by chicago department of health. Priority 7-38-005 citation issued.

- comments: observed over 100 mice droppings scattered on shelving units, prep tables, fryer cabinets, on floor around water heater, behind coolers, inside unused walk-in cooler. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed food items stored on floor in prep and storage areas. Instructed manager to elevate all food items off floor.

- comments: observed accumlated grease buildup and food debris on interior and exterior surfaces of cooking equipment, shelving units, prep tables, holding units, coolers, and freezers. Instructed manager to clean and maintain.
