- comments: observed no certified food managers certificate posted. Premises serves tcs foods: fish, sliced tomatoes, milk etc. Instructed the person in charge they must provide and maintain, post certificate visible to customers. Citation issued. Priority foundation 7-38-012. Citation issued.

- comments: observed ice scoop laying on top of ice machine. Instructed the person in charge to store ice scoop in a clean and sanitized container to prevent potential contamination.

- comments: observed accumulated grease on interior and exterior surfaces of all cooking equipment. Instructed the person in charge to clean and maintain. (nonfood contact surfaces)

- comments: observed outside garbage dumpster area overflowing , preventing lids from closing properly. Instructed the person in charge to ensure that trash is properly contained,not overflowing and that garbage lids are tight fitting. Priority foundation violation 7-38-020(b). Citation issued.

- comments: observed vent covers in both toilet rooms with dust accumulation. Instructed the person in charge to remove dust and maintain.

- comments: observed food debris/spills on floor throughout rear preparation area. --- also observed accumulated grease and food debris underneath cooking equipment in rear preparation area. Instructed the person in charge to detail clean, sanitize, and maintain.

- comments: observed one certified food manager with expired allergen awareness training certificate. Instructed the person in charge to renew expired allergen awareness training certificate and ensure that any individual that holds a city of chicago food sanitation certificate receives allergen training and allergen awarness certificate.
