- comments: observed 2 prep areas without required hand sink. Must install a dedicated hand washing sink in refrigerated room (next to meat counter) with prep equipment, such as meat grinder; must install hand sink in area with machine used to grind masa in basement. Must install dedicated, exposed hand washing sink with hot/cold plumbed water, soap/paper towels, and a hand washing sign in these areas. Priority foundation violation 7-38-030(c). Citation issued.

- comments: observed no hand washing signs or posters near hand washing sinks behind meat counter. Must provide and maintain.

- comments: visible and working thermometer needed in meat display cooler. Must provide and maintain.

- comments: must properly labeled food storage containers when food is not in the original containers.

- comments: observed a 1/2' gap along the bottom of front entrance door. Must provide a tight fitting seal at all outer openings tp prevent the entry of pests.

- comments: observed wiping cloths stored on top of prep tables behind meat counter and deli department. Must properly store wiping cloths in a sanitizing solution when not in use.

- comments: observed milk crates stored and used for product elevation in the hallway between meat counter and rear dry storage areas. Instructed to remove and provide approved shelving for product elevation.

- comments: observed heavily stained cutting boards in meat prep areas. Must resurface or replace the cutting boards in this areas.

- comments: observed leaking faucet base at the 3-compartment sink located in the basement dish washing area. Must repair and maintain.

- comments: observed clutter and unnecessary items in rear storage areas and in the rear of basement. Must remove clutter clean and organize area to prevent rodent harborage.

Observed standing water on floor next to unused walk-in freezer in basement. Must maintain floor dry at all times to prevent pest breeding.

- comments: observed mop stored in the mop bucket in basement. Instructed to properly stored mop after use.

- comments: previous core violations not corrected from report # 2456387 on 11/06/2020 #55 observed peeling paint on ceilings and walls throughout basement. Must removed peeling paint, and make surfaces smooth and easily cleanable. Observed missing/damaged ceiling tiles on west side of store, near door to back hallway. Must replace/repair. Must maintain same. #56 must provide adequate lighting in basement near rear stairs to properly clean area. Must remove dead bugs from light shields in bakery. Priority foundation violation 7-42-090. No citation issued.
