- comments: no written employee health policy on site for all employees. Must provide. Priority foundation violation.

- comments: the menu does not disclose or remind consumers of the risk of consuming raw or undercooked seafood. Must provide for menu. Priority foundation violation.
37. Food properly labeled; original container
- comments: all bulk food containers must be labeled with the product common name.
38. Insects, rodents, & animals not present
- comments: observed live roach and fly activity in numerous locations. Approximately 8 to 10 adult and baby roaches in the front sushi prep, in open crevices of raw wooden shelving above the two-compartment sink, in between the wall and plastic wall guard in the rear kitchen. Noted live small flies throughout the rear dish/prep, washroom, front sushi prep. Recommend having pest control co. Service establishment for flies and roaches. Must eliminate all pest activity. Priority foundation violation 7-38-020(a) citation issued.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: noted a bulk food container plastic lid broken and repaired with duct tape. Instructed to replace.

- comments: noted the exterior of the front such prep garbage containers dirty and badly stained. Instructed to clean and maintain. ----raw wooden shelving and unfinished absorbent wall above the sushi prep two-compartment sink. Raw wooden blocks used to prop sushi prep cutting boards. Must paint/seal to be smooth and cleanable. ----all open crevices in wooden shelving at the front sushi prep must be sealed. ----black plastic cart at rear prep shelving dirty. Plastic shelving in the washroom hallway is dirty. Must clean and maintain all. ----must not use cardboard or paper towels as liners for shelving throughout.

- comments: peeling plastic sealant on the top of the ice machine must be removed.

- comments: unused cooler on the rear outdoor area must be removed to prevent rodent harboring.

Floors in the rear dry storage at the water tank with debris. Must clean and maintain all. ----wall throughout the rear prep in poor repair with open crevices, peeling open gaps. Must seal.
