- comments: no city of chicago certified food manager on site during initial inspection. Priority foundation violation. No citation issued.

- comments: no city of chicago certified food manager on site during initial inspection while tcs foods were being prepared and served. Manager arrived one hour and five minutes after inspection began. Reviewed a city of chicago certified manager must be on site at all times tcs foods are being prepared and served. Priority foundation violation 7-38-012 citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: no written employee health policy on-site or available for for review for all employees. Must provide. Priority foundation violation 7-38-010. No citation issued at this time.

- comments: noted employees beverage on the cooks line prep cooler. Instructed to provide a designated area away from food preparation for employees refreshments.

10. Adequate handwashing sinks properly supplied and accessible
- comments: no paper towels located at the hand washing sinks located in the basement food prep area and the bar where customers were noted eating and drinking. Management provided towels during inspection. Paper towels must be available and accessible at all hand washing sinks at all times. Priority foundation violation 7-38-030(c) citation issed.

- comments: observed prepared tcs foods held longer than 24 hours such as cooked chicken, cooked macaroni and cheese, beef stock noted throughout several coolers and walk-in coolers without an expiration date. Some foods noted with preparation dates only and others with no date labeling. Instructed all prepared tcs foods held longer than 24 hours must be date labeled with an expiration date no longer than 7 days. All foods discarded. Priority foundation violation 7-38-005 citation issued.

- comments: noted an open mesh bag of soiled linens stored on the outdoor patio. Must properly store soiled linens in a sealed container.

- comments: extremely dirty interior of the basement employee washroom bathtub. Tub noted with dried, encrusted waste water debris. Must clean and maintain.

- comments: observed excessive amounts of food containers, food waste, bags of garbage on the grounds surrounding the outside waste containers. Instructed to remove all waste from the ground surrounding the containers and maintain clean. Priority foundation violation 7-38-020(b) citation issued.

55. Physical facilities installed, maintained & clean
- comments: unused ice machine in the kitchen, unused dirty coffee pots and microwave oven in the east basement dry food storage and all unnecessary items throughout the west side basement must be removed from the premises. Needed items throughout the basement must be organized and elevated.

Must clean and maintain all. ----missing wall at the eastside kitchen rear prep. Must replace. ----metal floor of the west kitchen rear walk-in cooler in poor repair. Noted buckled with open/broken seam. Must repair to be smooth and cleanable.

- comments: no proof of food handler training or certificates for all required employees. Must provide.

- comments: observed two employees (cooks) in the kitchen not wearing face coverings properly. Employees were noted wearing their masks under their nose, exposing their noses. Employees must properly wear face coverings covering both their nose and mouth. Citation issued 7-28-060.
