- comments: observed no signed employees' health policies for all employees must be provided and maintained. (priority foundation 7-38-010)(citation issued)
8. Hands clean & properly washed
- comments: observed poor hygienic practices by a food handler. Observed female food handler talking on cell phone. She then put on plastic gloves and handled bread to make a sandwich for a customer without washing her hands after every possible contamination. (priority 7-38-010)(citation issued)(cos)

- comments: observed improper temperatures of tcs ready-to-eat foods, such as 3lbs. Sliced tomato-45. 1f on topside of cold holding unit. The product was discarded by manager. Must have cold holding foods at 41. 0�f or below. Appx. 3lbs. $10. (priority 7-38-005)(cos)(citation issued)

- comments: must label food storage containers when food is not in original package (shakers, squeeze bottles).

- comments: observed evidence of an infestation of appx. 20 or more live roaches crawling along door gaskets of walk in freezer, underside of rear prep tables crevices, crawling on topside of walk in freezer amongs clutter, on upper wall shelving unit of receiver, on floor underneath rear prep table amongst food debris and clutter, on floor under cold holding unit on serving line and several dead on floor inside of walk in cooler & freezer, on floor underneath prep table and front 3- shelf counter near register. Must have a pest control operator to service premises on or after 11/14/25. (priority 7-38-020(a)(citation issued)

- comments: observed a gap along bottom of front door must make tight fitting.

- comments: must provide a splash guard at hand sink next to meat slicer or remove meat slicer to another location.
