2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs are being prepared,handled and served such as sliced (luncheon meats,rice,beans,etc). Must be on site at all times. Manager arrived on site at 9:40 a. M. (cos)(priority foundation 7-38-012)(citation issued)
36. Thermometers provided & accurate
- comments: must provide a thermometer for hot holding unit.
37. Food properly labeled; original container
- comments: must label food storage containers when food is not in original package.
38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 40 or more mice droppings on floor under shelving units and along wall bases in electrical room and on shelving unit in rear food storage area. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service on or after 3/10/22. (priority foundation 7-38-020(a)(citation issued)

- comments: must repair or replace an appx. '1/2-3/4' gap along bottom of front entry & exit door,rear overhead and man door. Must repair and maintain.

- comments: must provide a splash guard at hand sink next to 3- compartment sink in rear prep area.

- comments: must invert multi-use utensils on dish storage shelves.

- comments: must remove rust from lower shelves of prep tables. Must elimates crates used for food storage throughout premises. Must repair or replace loose kick plate on spice display,missing fan guard covers in meat walk in cooler. Must remove excessive caulking inside of cuber in meat dept.

- comments: observed worn and pitted cutting boards in meat prep area. Must repair or replace.

- comments: must clean debris build up from rolling carts,fanguards and pop sleeves in all walk in coolers.

- comments: observed missing faucet nor sprayer at large compartment sink used for large equipment to be washed,rinsed and sanitized, using an unapproved hose sink is not being used at this time. Must install a faucet and sprayer at sink and maintain.

- comments: observed outside garbage dumpster lids in poor repair on 2 of the 3 and encrusted grease on lid of grease container. Must repair or replace lids to be tight fitting and clean debris build up from grease container and maintain. (priority foundation 7-38-020(b)(citation issued)

- comments: must repair or replace damaged and broken floor tiles in walk in coolers,sales floor,storage areas and where needed,damaged wall on sales floor at produce area,missing wall bases throughout all prep areas. Must grout floors throughout all prep and dish washing areas,walk in coolers. Must remove black mold build up on caulk above 3- compartment sink in main prep area. Must scrape and paint peeeling paint on floors in all prep and walk in coolers.

- comments: must clean debris build up on walls and ceilings in all walk in coolers and men's toilet room.

- comments: must replace burntout lights in all prep and dish washing areas.

- comments: previous core violations from report #2521791 6/23/21 not corrected #51- missing stoppers for 3-compartment sink. Instd to provide and maintain. #56-missing ventilation for bread oven (11kw). Instd to provide ventilation or remove. #57-food handlers with expired training certificated.
