3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: no written employee health policy on site for all employees. Must provide. Priority foundation violation 7-38-010 citation issued.

25. Consumer advisory provided for raw/undercooked food
- comments: menu does not disclose and inform consumers the specific menu items such as burgers that are raw or undercooked. Must disclose and remind customers of such food on both the printed and virtual menus. Priority foundation violation.

37. Food properly labeled; original container
- comments: all bulk foods not in the original container including sprays, oils and spices, must be labeled with the product common name.

38. Insects, rodents, & animals not present
- comments: rear exit door to the alley with an open 1/2 inch gap on the bottom threshold with a torn strip. Must replace strip and provide a tight fitting seal on the door.

39. Contamination prevented during food preparation, storage & display
- comments: observed foods being prepared ie: fry slicer with piled potatoes and onions in an area of the basement not protected from overhead wastewater piping. A non-commercial ventilation system has been installed in the same area. Also noted is a box freezer under wastewater piping and open cases of potatoes under the same lines. Instructed to remove all food preparation items from the basement, remove the non-commercial ventilation or install smooth floors, walls, and ceiling throughout out. Priority foundation violation 7-38-005 citation issued.

40. Personal cleanliness
- comments: all employees with beards must wear a beard restraint.

41. Wiping cloths: properly used & stored
- comments: soiled wiping cloths must be held in a container with a sanitizing solution.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must not use raw wood as a means of elevation for the broken wheel leg on the front cooler. Must replace.

49. Non-food/food contact surfaces clean
- comments: shelving under flat top grill with excessive food debris. Must clean and maintain.

51. Plumbing installed; proper backflow devices
- comments: leak on the nozzle of the one compartment prep sink in the rear prep/dish area. Must repair.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: must provide a covered waste receptacle in the unisex washroom.

- comments: must provide a waste receptacle at the prep hand washing sink.

- comments: floors throughout the front prep area behind coolers and under shelving with dirt and debris. Must clean and maintain.
