- comments: observed ready to eat, temperature control for safety foods (cream cheese, deli meats, spicy mayo )not properly dated , stored longer than 24 hours in coolers. Instructed to provide proper date marking with disposition date for rte foods prepared on site and held over 24 hours in cold holding unit. Priority foundation violation. No citation issued. 7-38-005

- comments: observed no consumer advisory disclosure or reminder on menu for use raw and under cooked foods. Must provide and maintain. . Priority foundation-7-38-005. No citation issued

- comments: observed that pest control log have incomplete pest control record. Book is missing required information. Missing record of visits, missing copy of service tickets. Must provide and maintain all required pest control records and documents. Priority foundation violation. 7-38. 020 (c) citation issued

- comments: observed exposed sewage pipes above basement food storage. Overhead exposed drainage waste water pipes coming from the 1st floor areas. The said pipes where noted to manager must be encased smooth surface (soffit) durable material and maintain

- comments: observed wet wiping cloths stored on top of the prep tables. Management instructed to store wet wiping cloths in a container of sanitizer.

- comments: noted some use of milk crates for elevation at the kitchen area, dishwashing area and dry storage area. Instructed not to use milk crates for elevation to prevent hiding places for pest. Must provide appropriate shelving for storage. All food items must also be stored 6' away from the wall

- comments: noted no sanitizer test strip kit on site at the time of inspection to check concentration of sanitizer solution on 3 compartment sink. Instructed to have a test strip kit on site at all times to properly monitor the strength of the sanitizer. Priority foundation. 7-38-005 citation issued

- comments: backflow prevention devices not located at ice machine located in basement of facility. Must install so backflow prevention devices or air gap between the water supply inlet and the unit so it may be located to be serviced and maintained.
