- comments: 4-204.112(b) out c 07/23/2026 observed multiple cooling units (dairy, sourcream, cheese, deli meats, breakfast sausages, bacon, salsas and guacamole) etc in grocery without thermometers inside. Instructed to provide and maintain.

38. Insects, rodents, & animals not present
- comments: 6-501.111 observed approximately 100 or more small and large rodent droppings on premise scattered on floor near deli (bread display), in rear warehouse behind meat prep/dairy cooler, on 2nd floor in storage on pallets containing dog/cat food and charcoal, scattered on floor in liquor storage and in between pallets throughout storage connected to prep area. Noted two dead mice on traps inside bait boxes near waste receptacle. Instructed to contact pest control for additional storage, remove droppings, clean and sanitize said area. Must eliminate rodent activity on site.

- comments: 4-501.11 observed torn gaskets on 2 door refrigeration unit located in meat and sandwich/salad prep area. Instructed to replace and maintain.

- comments: 4-601.11(c) observed cooling units containing produce as well as sauces with dried food debris on surfaces. Instructed to clean and maintain.

- comments: 6-201.17 observed water stained and damaged ceiling tiles throughout grocery and prep areas. Instructed to replace and maintain smooth and easily cleanable attachments.

- comments: 6-201.11 observed wall located in rear storage area with drywall removed and surface not yet finished or smooth and easily cleanable. Instructed to enclose open space to prevent pest entry and provide smooth and easily cleanable surfaces.
