- comments: not all employee signed 'employee health policy'. Instructed to provide. Priority foundation violation 7-38-010. No citation issued.

- comments: observed employee handling non-food items,example dirty door handled from walk-in cooler,with a rag trying to remove house flies from top of prep table,handling cardboard boxes etc,than returned at pizza work area,handled dough and condiments. No hand washing prior to handle food and no gloves. Food handlers must wash hands and change gloves after any possible contamination. Information given to mmager on hand washing and glove procedure. Manager discussed issue with employee. Priority violation 7-38-010. Citation issued. .

- comments: no handwashing sink installed at pizza area, also front prep and serving area where raw,cooked and ready to eat food products are handled,cooked and served. Priority foundation violation :7-38-030(c), no citation issued citation issued.

- comments: observed approx 40 mice droppings at pizza prep area under and on top shelves,20 small flies at the rear storage area by the onions, ten or more house flies throughout the premises.
