- comments: 2-102. 14(n) noted no employee health policy on site at the time of inspection following the new code and guidelines. Instructed to have verification of policy for each employee one on site at all times. Priority foundation violation7-38-010. Citation issued

- comments: interior surfaces of ice machine not clean. Black mold-like substance on sides of the bin. Rust and mineral deposits build up on upper surfaces and ice chute. Instructed management not to use ice machine until equipment is detail cleaned and sanitized. Priority foundation violation 7-38-005. Citation issued.

- comments: 6-501. 111 noted over 100 mice droppings on floor around behind the meat counter, underneath the 3 compartment sink/dry storage area by the meat counter, rear dry storage on shelf with bags of salt and at the front display area where dry is stored. Instructed to detail clean and sanitize affected areas, eliminate pest activity. Instructed to consult with pest control company and have premises treated. Priority foundation 7-38-020(a)

- comments: food containers stored on floor in the meat prep counter, walk in coolers and dry storages. Must provide proper, raised shelving to store food containers 6" off floor.

- comments: noted ice build up inside the walk in freezer on the floor and shelves. Instructed to repair and maintain the condensation units.

- comments: noted some use of milk and soda crates for elevation throughout establishment. Instructed not to use milk or soda crates for elevation to prevent hiding places for pest. Must provide appropriate shelving for storage. All food items must also be stored 6" away from the wall.

Food boxes must be stored 6" off the floor for easy access on cleaning. -43)3-304. 12:instructed to provide proper knife rack or magnetic strip for proper knife storage in rear produce prep area. -47)4-101. 19:instructed to detail clean and maintain storage metal racks and vegetable display racks with mold like build-up inside cold holding display cooler in rear display area. -49) 4-601. 11(c):observed heavy ice build-up on condenser fan cover and ceiling inside walk-in freezer in rear walk-in freezer. -53)5-501. 17:toilet room receptacle/trash bin must be covered.
