1. Person in charge present, demonstrates knowledge, and performs duties
- comments: the person in charge at the time of inspection does not have a city of chicago sanitation certificate. Priority foundation violation. No citation issued -

2. City of chicago food service sanitation certificate
- comments: no certified manager onsite with a valid city of chicago food sanitation license posted. Instructed to have a certified manager on-site with a valid city of chicago food sanitation license at all times when tcs foods: sausage, eggs, milk, etc. , are being prepared or served. Priority foundation 7-38-012 citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: found no vomit and diarrhea clean-up kit onsite during the inspection. Priority foundation, 7-38-005 citation issued. Must provide.

10. Adequate handwashing sinks properly supplied and accessible
- comments: no exposed sink is installed in the rear prep area were raw, cooked, and ready-to-eat food is prepared. Instructed to install with hot/cold running water, also provide soap and paper towel/hand drying device at said sink. Priority foundation violation:7-38-030(c), no citation issued.

- comments: label food stored inside coolers with a common name.

41. Wiping cloths: properly used & stored
- comments: must store wiping cloths in prep areas in sanitizer bucket between uses to prevent cross contamination. Instructed to provide

- comments: must not use milk crates as a means of elevation for stored items.

49. Non-food/food contact surfaces clean
- comments: grease spillage at exterior of stove, clean and maintain

50. Hot & cold water available; adequate pressure
- comments: no running hot water is provided at washbowl inside the staff/residents' washroom first floor. Only cold water is provided. Another washroom is available with hot water at 100. 5f. Hot water temperature at handwashing sinks must be at a minimum of 100. 0f or higher through a mixing valve or combination faucet no more than 120f. Priority violation. 7-38-030(c). Citation issued. Instructed to repair issue.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: cover is needed at receptacles inside the staff washrooms

55. Physical facilities installed, maintained & clean
- comments: holes on the ceiling and walls in the kitchen around pipes. Instructed to repair. The walk-in cooler is not easily cleanable(raw wood panels). The surface must be smooth, cleanable and non-absorbent material

61. Summary report displayed and visible to the public
- comments: previous summary report not posted on-site from 10-2-2019, report#2315237. New summary report given, instructed to post on site visible to customers. Priority foundation:7-42-010(b)citation issued.
