I woke up at 6am with gut wrenching vomiting profuse diarrhea every part of my body aches and shooting pains throughout my body. One of two culprits: Costco Lehi,UT OR Smith's Lehi, deli Sushi

Nov 6, 2018: Tuesday evening about 4:00 pm - purchased sushi plate from Smith's, Lehi, UT. Refrigerated overnight. Wife eats a separate sushi bowl. (wife does not become ill)

of the sushi plate at about 2pm. I eat remainder of sushi plate about 5pm.
Nov 8, 2018, Thursday: Daughter and I both eat a Kirkland Brownie protein bar purchased form Lehi, Costco approximately two weeks prior.
9:44 pm daughter begins feeling nauseous; throws up all night; diarrhea goes with it.
2:00 am I feel sick; feel unwell most of the night and vomit 8 to 10 times within a 30 minute period around 7 am on November 9.
Wife never gets ill.
Vomiting and diarrhea were severe. Illness resolves within 24 hours.  | Symptoms: Diarrhea, Fever, Nausea, Vomiting

sandwich with bacon and a bunch of stuff on it... Not at all what i ordered. I turn around go inside this time and wait a good 10 minutes while 4 employees stand around (fully aware i am holding an unwrapped sandwich) and proceed to tell the only woman doing her job that the sandwich was incorrect. She took my order again they remade my sandwich and i left. Went back to work (late because i had to exchange my food) and ate lunch. 4 hours later I receive a severe migraine nausea accompanied woth vomiting diarrhea stomach pains which eventually led to blacking out. I have never had such bad food poisoning in my entire life. Im not saying that the employees did something to my sandwich... but it wouldnt be surprised. Please look into the cleanliness and the food preparation of this Wendys location.  | Symptoms: Diarrhea, Fever, Nausea, Vomiting, Stomach Pain, Migraine

I ate the vegetarian sandwich at around 9 pm (white bread, tofu, greens, tomato, guac, no pesto, no cheese), started feeling nauseous not long after. I tried going to bed about 10:30 but tossed and turned with a fever and nausea until vomiting it all up at 2am.  | Symptoms: Fever, Nausea, Vomiting

I ate there on July 2nd around 7:30 with my husband. I ordered the pork ruban (part of their summer specials) I woke up around 1:30 am and vomited.
