Got 2 largge pizzza's, one with 2 toppings, one with 5. They both seemed undercooked, but the center of the 5 topping pizza was really doughy. I ate 2 pieces.

1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed no person in charge with a city of chicago certificate. Instructed to have a certified food manager with the city of chicago certificate on site at all times.

- comments: observed no city of chicago certified food manager on site during the inspection while the temperature control for safety foods (beef,rice,beans etc. ) are prepared and served to customers. Instructed a city of chicago certified food manager must be on site at all times foods while (tcs foods) are being handled prepared and served. Priority foundation violation 7-38-012,citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: instructed to provide and maintain handwashing signage at handsink at rear prep/dishwashng area.

- comments: observed (tcs foods) at improper cold holding temperature inside of the walk-in- cooler. - comments: observed no consumer advisory disclosure on the menu for raw or undercooked foods. Instructed to disclose or describe which foods can be ordered raw or undercooked and link those items to the reminder statement on the menu. Must include the following sentence before the reminder statement:'these foods can be ordered raw or undercooked or may contain raw or undercooked ingredients. ' priority foundation violation 7-38-005.

- comments: observed bulk containers in rear without a label. Instructed to provide the common name on all bulk containers to properly identify item (salt, sugar, flour etc).

- comments: backflow prevention device not located at coffee/espresso machine. Must install so backflow prevention device may be located to be serviced and maintained.

- comments: instructed to detail clean kitchen exhaust hood/filters to remove accumulation of grease build-up and to maintain.

Instructed the person in charge to identify such foods by asterisking them to a footnote that states: 'items can be served raw or undercooked or contain undercooked ingredients' and add a statement that reads: 'consuming raw or undercooked animal products may increase your risk of food borne illness, especially if you have medical conditions. 'priority foundation 7-38-005. No citation was issued.

- comments: observed approximately 20 mouse droppings along the wall underneath the two-compartment sink located in the rear preparation area. Instructed to remove all rodent droppings, clean surfaces, and maintain premises free of pests. Recommended receiving additional pest control service. Priority foundation violation 7-38-020(a). Citation issued.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed both toilet room doors not self-closing. Instructed the person in charge to provide a self-closing mechanism for both toilet rooms doors.

- comments: observed grease buildup, food debris, and dirt on the floor under cooking equipment, along walls, and in all corners in prep, and dishwashing, areas. Instructed the person in charge to clean floors under all equipment, along walls, and in all corners in all areas.

56. Adequate ventilation & lighting; designated areas used
- comments: observed one burnt-out lightbulb underneath the hoodedvent, located in the rear preparation area. Instructed the person in charge to replace burnt-out lightbulbs and maintain adequate lighting.

58. Allergen training as required
- comments: observed one certified food manager without allergen awareness training. Instructed the person in charge to ensure that any individual that holds a city of chicago foodservice managers certificate obtains allergen awareness training.
