Observed over 10 small flies on the wall at the rear of the service counter behind the fresh squeeze orange juice machine. Observed 5 live small flies inside a fly trap device on the counter. 3. Observed over 10 live small flies flying and landing on surfaces in the cabinet under the hand sink at the front counter and 3 small flies on plastic bins storing single use items in the cabinet to the right of the hand sink cabinet. 4. Observed over 5 small flies flying above a floor drain under the three compartment sink. 5. Observed 1 small fly on a bakery board on a shelf in the bakery prep area. Employee moved board to the ware washing area to be cleaned and sanitized. Operator began killing the flies and cleaning and sanitizing the areas. **Repeat Violation**
High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Observed panned frozen chicken stored above baked goods in the walk-in freezer. Employee rearranged items for proper storage. **Corrected On-Site**
High Priority - Roach activity present as evidenced by live roaches found. Observed 1 live roach on the wall near the ceiling above the dough mixers. Employee killed the roach and cleaned and sanitized the area. **Corrective Action Taken**
High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed in the Hot display case; beef empanada (123°F - Hot Holding); chicken empanada (112°F - Hot Holding); spinach empanada (115°F - Hot Holding). Per the employee the items were placed in the case 2,5 hours prior. Employee increased the temperature of the case. **Corrective Action Taken**

Observed 2 small flies on an empty box of liquid frying oil on the floor under a prep sink to the left of the three compartment sink. Observed 2 small flies on the wall behind a storage shelf containing food storage pans, plates, and boxed single service items to the left of the three compartment sink. Observed 2 small flies on a shelf to the left of a prep table containing imitation flavorings used in baking. Observed 1 small fly on cream cheese in a self-service display case. Observed 1 small fly crawling on the outside of the fresh squeezed orange juice machine and one in a glass of freshly squeezed juice. Operator discarded the juice. Operator began killing flies and cleaning and sanitizing the areas during the inspection. - From follow-up inspection 2021-09-15: At callback inspection, 1. Observed 3 small flies on the wall at the front counter near the hand wash sink and coffee machine. Operator killed the flies and cleaned and sanitized the areas. 2. Observed 3 small flying insects near a storage shelf containing imitation flavorings for baking and 1 on the wall behind the same shelf. Operator killed the fly on the wall and cleaned and sanitized the wall and all items on shelf. Observed no more flies after this cleaning. **Time Extended**
