Also, the person in charge must have a city of chicago certificate by next inspection (upon being inspected) priority foundation violation 7-38-012. Citation issued.

13. Food in good condition, safe, & unadulterated
- comments: noted heavy build-up on the condensation unit and shelves dripping onto food items stored on shelves inside the freezer. Manager immediately and voluntarily disposed of 10 lbs of products worth $40. 00 through denaturing process. Instructed defrost and repair the condensation unit and maintain. Priority violation #7-38-005. Citation issued

38. Insects, rodents, & animals not present
- comments: noted over 300 mice droppings throughout the food dry storage area on shelves, inside boxes, on the floor along the wall base, at the rear kitchen food prep area on the floor, food carts, and shelves. Also noted was gnawing into food containers (cups) on shelving at the food dry storage. Instructed to detail clean and sanitize affected areas eliminate pest activity. Instructed to consult with pest control company and have premises treated. Priority #7-38-020(a). Citation issued.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: noted some use of milk crates for elevation in the kitchen area and dry storage area. Instructed not to use milk crates for elevation to prevent hiding places for pest. Must provides appropriate shelving for storage. All food items must also be stored 6' away from the wall.

- comments: noted rusty and dirty shelves inside the walk-in cooler at the kitchen prep area. Instructed to detail clean, repaint with toxic food grade paint, and - or replace.

- comments: instructed to detail clean and sanitize shelves at the food storage area, carts, trays, and where ever needed that mice dropping were found.

- comments: noted leaking faucets at the handwashing sink by the dishwashing area, 2-compartment sink, and 3-compartment sink. Instructed to repair and maintain.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: noted washrooms on premises with garbage receptacles and no lids. Instructed to provide trash cans with lids at all women washrooms and unisex washrooms

55. Physical facilities installed, maintained & clean
- comments: instructed to detail clean floors along the wall base and underneath equipment, especially where the droppings were found. Must upgrade housekeeping.

- comments: observed no vomiting and diarrhea procedure or clean-up kit on premises during the inspection. Instructed to have said procedure along with required equipment for clean-up on site at all times. Priority foundation violation 7-38-005, citation issued.

- comments: observed no handwashing signage at the handwashing sink in the food prep area across from the ventilation hood. Instructed to provide said handwashing signage and maintain.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed excessive black slimy substance build up on inside interior panel of the ice machine and dripping onto ice for human consumption. Instructed the manager they must remove the build up, clean, and sanitize. Also, instructed them not to use the ice currently inside of the ice machine. Priority foundation 7-38-005, citation issued.

38. Insects, rodents, & animals not present
- comments: observed dead roaches throughout the food prep area on the floors in the following areas: under the three-compartment sink, on the walls throughout the facility especially above the three-compartment sink, and on the sticky traps throughout the food prep area. Instructed to remove all dead insects in the food prep area, clean, sanitize, and maintain.

38. Insects, rodents, & animals not present
- comments: observed over 15 mice dropping on ventilation system, floors, and boxes in the dry storage area of the facility. Also observed one live roach on the soap dispenser in the dishwashing area. Recommended to consult with the pest control company prior to being re-inspected. Must also completely remove the droppings, clean, and sanitize the affected area and also limit the roach activity on site prior to being re-inspected. Priority foundation #7-38-020(a), citation issued.

- comments: observed the facility using the hot water from the coffee machine in the food prep area for the mop bucket. Instructed to utilize the utility sink/mop sink to avoid possible contamination.

There was kitchenware (spoons) and severe debris build up at the area where the dishes were stored and on the side of the ice machine and under the three-compartment sink. Instructed to clean, sanitize and maintain said floors.

- comments: observed damaged filters creating an opening in the filters on the ventilation hood. Instructed to repair said filters and maintain.

60. Previous core violation corrected
- comments: previous core violations not corrected from10/08/2019 , report #2315481 #55 found floors not clean under shelving units at dry storage areas. Must clean and sanitize. #found no ceiling exhaust for employee restroom. Must provide. Priority foundation 7-42-090, citation issued

that are the subject of this warning were offered for sale only at Fleur de Sel located at 601 Rue Principale, Granby, QC, Canada. The issue was discovered during routine checks by the MAPAQ. As of now, no allergic reactions related to the consumption of these products have been reported.

The recalled products include "Filet de Sole Stuffed with Salmon and Shrimp", "Cod Loin Saffron and Chives", and "Basil and Tomato Chicken". These items were sold frozen and vacuum-sealed in clear plastic bags. Specific details such as brand, size/weight, batch, UPC, and best before date were not provided, nor was the amount of recalled products. The products were vacuum-packed in a transparent plastic bag.
