- comments: observed evidence of appx. 20 or more live roaches crawling on under side and wall at 2- compartment sink,on floor behind cooking equipment,on wall and shelving units in dish washing area,inside of panel at ice machine in hallway. Also, evidence of appx. 5-7 live small flies underneath ice machine in hall way. Recommended to have a pest control operator to service premises on or after 6/18/21. (priority foundation 7-38-020(a)

- comments: must store wiping cloths in a sanitizing solution when not in use.
