- comments: found tcs foods at improper temperature: 15lbs of slice turkey at temp of 49. 6f to 53. 5f; 75each 1oz of cream cheese at temp 49. 7f. Cold food must maintain a temp of 41f and below. Priority violation: 7-38-005,citation issued

- comments: two-door reach-in cooler maintains air temp of 49f to 51f. Unit must maintain air temp of 41f and below. Priority violation;7-38-005,citation issued

- comments: bulk food stored in plastic container in the kitchen must be labeled with a common name
38. Insects, rodents, & animals not present
- comments: we observed over 40 mice droppings in different locations of the kitchen and storage area: under shelves and crates in food storage, behind cooking equipment in the kitchen, and under a three-compartment sink(north-east corner). Mice droppings were observed rear area by the walk-in freezer and unused equipment. Instructed to remove droppings, clean and sanitize all areas. Priority foundation violation:7-38-020(a), citation issued

- comments: outer opening not protected. Exit rear bottom door has a gap of 1/4'. Instructed to repair issue. To minimize pest entry

- comments: broken rubber gasket inside the two-door of reach-in cooler in the kitchen. Also broken interior and exterior door liner at walk-in cooler. Repair/replace
