- comments: observed employees drinking coffee and water from open cup in prep area. Instructed manager to provide closed lid drinking containers for employees.

- comments: observed no hand washing signs at hand washing sinks in serving area and rear staff washroom. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

- comments: observed fish improperly thawing on bottom of prep tables. Instructed manager to properly thaw food under recrigeration, cold running water or part of cooking process.

- comments: observed three live roaches crawling on wall behind aquafina cooler, crawling on floor behind two-door reach-in cooler and crawling on floor near cooking equipment in stir fry prep area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed single service articles stored on floor in unused prep area. Instructed manager to elevate all single service articles off floor to protect from contamination.

- comments: observed the following food equipment and contact surfaces not clean: -interior/exterior of the deep fryers/cooking equipment with accumulated food, grease and dirt build up. -storage racks with food and dirt debris in the food prep area and inside the walk in coolers. -lower surface of prep tables -rubber gaskets on the walk/reach in coolers. Instructed to detail clean and maintain all.

- comments: observed floors with excess water, dirt, food, grease and debris through out including the walk in coolers/freezers. Instructed to detail clean and maintain floors through out. -walls and ceiling with food, dirt and grease build up in all prep and storage areas, around sinks and heavy equipment. Instructed to detail clean and maintain all.

- comments: observed no valid allergen training for certified managers. Instructed manager to provide.

- comments: previous core violations not corrected from report #255908 on 5-17-22: 35-observed raw shrimp improperly thawing at room temperature. Instructed manager to properly thaw under refrigeration or cold running water. 37-observed bulk containers not labeled in prep and storage areas. Instructed manager to label all bulk containers with common name of food. 39-observed food items stored on floor. Instructed manager to elevate all items six inches off floor. 41-observed wiping cloths not properly stored when not in use. Instructed to store in sanitizing solution (must be at the required ppm) when not in use. 53-observed no covered waste receptacle in women and staff unisex washroom. Instructed manager to provide. 55-observed damaged floor tiles and missing grout on floor tiles in prep, storage and dish washing areas.
