- comments: observed no person in charge with valid chicago food sanitation certificate. Instructed manager to designate person in charge during all hours of operation. Priority foundation

- comments: observed no chicago food sanitation manager on site or valid certificate posted when cooked beef (173. 5f) prepared and served. A chicago food sanitation manager must be on site at all times tcs food prepared and served. Priority foundation 7-38-012 citation issued

- comments: observed open cup of coffee on prep table in meat prep area. Instructed manager to provide closed lid containers for employee beverages.

- comments: observed no handwashing signs at handwashing sinks in staff and customer washrooms. Instructed manager to provide handwashing signs at all handwashing sinks used by employees.

- comments: observed uncovered raw beef stored above uncovered cooked pork in walk-in cooler, uncovered raw fish stored next to uncovered cooked vegetables in reach-in cooler. Food shall be separated and protected from cross contamination during storage. Priority 7-38-005 citation issued

- comments: observed bulk seasonings not labeled in meat prep area. Instructed manager to label all bulk containers with common name of food.

- comments: observed insect control devices hanging from ceiling meat prep area. Instructed manager to remove all insect control devices from meat prep area.

- comments: observed food items stored on floor in walk-in cooler and storage areas. Instructed manager to elevate all food items six inches off floor.

- comments: observed soiled wiping cloths stored on prep tables in hot food and meat prep areas. Instructed manager to store wiping cloths in sanitizing solution between uses.

- comments: observed styrofoam cup and bowl used as dispensing utensils. Instructed manager to provide long handle dispensing utensils to protect food from contamination. Observed ice scoop handle laying on ice inside ice machine. Instructed manager to properly store ice scoop handle upright to prevent contamination.

- comments: observed large multi-use pots stored on floor in rear prep area. Instructed manager to re-wash, rinse, sanitize all multi-use equipment and elevate all multi-use equipment and utensils off floor.

- comments: observed single service articles stored on floor in prep and storage areas. Instructed manager to elevate all single service articles six inches off floor.

- comments: observed rust deposits on interior and exterior surfaces of ice machine in second floor storage. Observed damaged door gaskets on liquor walk-in cooler door and reach-in cooler along east wall in sales area. Observed ice buildup in chest freezers in second floor storage area. Instructed manager to repair and maintain.

- comments: observed wood surfaces on multi-use equipment storage shelving. Instructed manager to seal all raw wood surfaces and maintain.

- comments: observed several large pots, containers and equipment too large to submerge in 3-compartment sink. Instructed manager to provide alternative method to properly wash, rinse and sanitize all large equipment.

- comments: observed low hot water pressure at handwashing sink in second floor staff washroom and at right handwashing sink outside washroom. Instructed manager to repair and maintain.

- comments: observed missing lids on outside garbage dumpster. Instructed manager to replace all missing lids on dumpster. Outside garbage dumpster shall be covered with tight-fitting lids. Priority foundation 7-38-020(b) citation issued

- comments: observed missing grout on floor tiles and damaged floor tiles near rear exit door in kitchen. Observed missing ceiling tiles in second floor storage area. Instructed manager to repair/replace all missing and damaged floor and ceiling tiles.

- comments: observed ventilation fan not operable in second floor staff washroom. Instructed manager to repair ventilation fan and maintain.

- comments: observed some expired food handler training for employees. All employees handling food and/or equipment must provide food handler training.

- comments: observed no allergen training for certified manager. All certified managers must provide allergen training.
