Report date: Dec 9, 20255. Procedures for responding to vomiting and diarrheal events
- comments: observed no written procedure posted or neccesary items outlined in policy available on site for responding to vomit and diarrheal events that can occur in the food establishment. Instructed to provide and maintain as required. Priority foundation violation. 7-38-005. Citation issued.
6. Proper eating, tasting, drinking, or tobacco use
- comments: observed food handlers drinking from can coke and opened coffee mugs stored on bakery table where items are prepared. Instructed to provide food employees a covering or lid on beverage to be consumed in designated area away from exposed food, equipment in order to prevent contamination.

- comments: observed handsink located in front prep area without hot running water. Instructed to provide and maintain as required at at least 100f. Priority violation. 7-38-030 (c). Citation issued.

- comments: observed no metal stem thermometer for testing interanl temperatures of tcs foods (milk, eggs, etc). Instructed to provide and maintain on site as required. Priority foundation violation. 7-38-005. Citation issued.

- comments: observed missing labels on gelatin located in front display cooler, instructed to provide and maintain
38. Insects, rodents, & animals not present
- comments: observed rodent infestation on premise during time of inspection. Observed approximatley 500 or more small and large rodent droppings in the following areas as follows. . . Upon entrance of facility in corners under shelving racks containing prepackaged bread, on all piping connected to plumbing fixtures, under prep table in rear prep scattered throughout shelving in between working materials stored on said shelves, along side of rolling carts/speed racks containing ready to eat foods trhoughout prep, on floor in all corners throughout front and rear prep area, scattered on shelves where vegetable oil, rice and other ingredients are stored alongside of all stand in place mixers and heavy equipment,inside toiletroom surrounding toilet, under mopsink and in corners, in rear storage area inside unused equipment, scattered on wood riser storing unopened flour, along floor in employee/storage space in basement along walls and equipment stored in said space. Noted approximately 3o small and large flies throughout front and rear prep, on exterior surfaces of equipment on plumbing fixtures in toilet room and on top ofcard board boxed used for liners throughout premise. Instructed to contact pest control for additional service, remove droppings, clean and sanitize effected areas. Must eliminate pest activity on premise. Priority violation. 7-38-020 (a). Citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: observed 7 speed racks storing ready to eat breads, assorted pastries stored in front of facility upon entrance without protection from contamination from consumers. Instructed to provide, packaging, display casing or other effective means to prevent possible contamination and maintain. Person in charge moved spped racks to behind cashier area inaccesible to public. Priority violation. 7-38-005. Citation issued.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed 2 door cooling unit door handles located in rear storage space of facility in disrepair. Instructed to repair and maintain in good condition.
49. Non-food/food contact surfaces clean
- comments: observed the following equipment in need of cleaning, they are as follows. . . Prep tables, stand in place floor mixers, all bulk storage containers, interior and exterior of refrigeration units, exterior ovens, rolling carts and speed racks throughout, baking pans and bread display cases. Instructed to remove built up dried food debris from surfaces, clean and sanitize.

- comments: observed large holes in floor in rear storage area as well as throughout prep near toiletroom. Instructed to provide smooth and easily cleanable surfaces and maintain.

- comments: bserved floors throughout facility with excessive buildup of trash, flour, food debris, clothing and other misecellane items. Instructed to remove, clean and santiize to prevent hiding spaces for rodents as well as feeding source. Must maintain.

- comments: observed excessive amounts of cardboard boxes, unused heavy equipment, wood palettes, clothing articles, bakery supplies unorganized stored improperly throughout rear storage space. Instructed to remove all uneccesary items and provide adequate shelving to store items at least 6 inches off floor to prevent pest harborage.

- comments: observed food handlers without food handler certifications as required. Instructed to provide and maintain on site for review.
