- comments: no person in charge during initial inspection. Employees on site without city of chicago sanitation certificates or knowledge of any required documentation.

- comments: no city of chicago sanitation certificates posted on site or a certified food manager on site during the inspection while tcs foods were being prepared and served. A certified food manager arrived at the end of the inspection. Reviewed that a city of chicago certified food manager must be on site at all times while tcs foods are prepared and served and certificates must be posted on the premises. Priority foundation violation 7-38-012 citation issued.

- comments: no written employee health policies on site for review. Must provide a written employees health policy verified by all employees. Priority foundation violation.

- comments: no written vomit/diarrhea cleaning procedure and all required equipment for such an event on site. Must provide. Priority foundation violation.

- comments: must install a hand washing sink with hot and cold running water, soap and paper towels at the rear area where a working oven, on/operating at the time of inspection, is being used or remove oven. Priority foundation violation.

- comments: observed the non-ice contact surface of the opening of the ice machine bin with excessive rust. Must remove rust and maintain.

- comments: observed several prepared tcs foods throughout coolers held longer than 24 hours such as cooked chicken, beans,flan,pork and stuffed poblanos with no dates. Other foods with preparation dates only. All foods discarded. Instructed all prepared tcs foods must be date labeled with an expiration date no longer than 7 days. Priority foundation violation 7-38-005 citation issued.

- comments: the business uses a virtual menu only that does not disclose and inform consumers the specific menu items that are raw or under cooked and a potential hazard of consuming such food(carne asada/steak a cook to order food) must disclose and remind such items. Priority foundation violation.

- comments: all bulk food containers, such as rice, beans etc. Must be labeled with the product common name.

- comments: no pest control log book on site available for review. Must provide a pest control log book with all required documentation for review. Priority foundation violation.

- comments: must not use wet paper towels as liner inside the cut lime container at the bar.

- comments: observed porous rock/stone bowls throughout the premises and being heated in a rear area oven to be used as an employee stated for food service of barbocoa and chicken. Must remove from the premises and must not use as food service dishware. Unable to properly sanitize porous rock.

- comments: noted the table top can opener with encrusted food debris. Must clean after each use.

- comments: all unused, unnecessary items stored throughout both rear storage areas must be removed to prevent rodent harboring and all other items must be stored elevated from the floors and organized.

Must properly repair/seal to be smooth and cleanable. ----noted water damaged and missing ceiling tiles throughout the rear storage area where clean pots, pans, cooler/freezers are located and in both washrooms. Must replace all.

- comments: no proof of food handler training or certificates on site for all required employees. Must provide.

- comments: no proof of allergen training or certificate for the certified food manager that arrived at the end of the inspection. Must provide allergen training for all city of chicago certified food managers.

- comments: previous core violations from 11-13-19 report #2345622 not corrected. #52- 5-403. 12- must provide a drain hoses attached to pop guns dispensers at bar area. Hoses must drain on floor drains. #55- 6-201. 11--cement floor at rear storage area uneven,not smooth not cleanable. Instructed surface must be smooth cleanable and non-absorbent surface. #47- 4-501. 11- must provide a lock for the walk-in cooler at hallway by washrooms excess to customers.
