- comments: observed no certified manager on site at time of inspection. Instructed to maintain an employee holding a valid city of chicago foodservice manager certificate on site at all times while open and operating. Priority foundation violation 7-38-012. Citation issued.

- comments: observed no verifiable health policy on site at time of inspection. Left template and instructed to maintain copies of health policy signed by all employees on site at all times. Priority foundation violation 7-38-010. No citation issued.

- comments: observed the following tcs items stored at the following improper temperatures inside a coca-cola cooler located in customer service area: kaddu ki kheer at 61. 5f, sweet bread 60. 4f, and yogurt based juice drinks at 60. 8f. Management voluntarily discarded approximately 4lbs of product valued at approximately $50. Instructed to maintain all tcs items at or below 41f at all times. Priority violation 7-38-005. Citation issued.

- comments: observed a coca-cola cooler located at front customer service area containing tcs items such as kaddu ki kheer, sweet bread, and yogurt based juice drinks maintaining an improper ambient temperature of 60. 2f. Cooler tagged and held for inspection. Instructed to maintain all coolers containing tcs items at or below 41f at all times. Priority violation 7-38-005. Citation issued.

- comments: observed no thermometer inside prep cooler located in rear prep area. Instructed to provide.

- comments: observed rodent droppings throughout the facility. Observed approximately 10 rodent droppings in storage closet located in women's restroom, 5 rodent droppings in closet located next to employee toilet room, and approximately 20 rodent droppings behind main cook line and underneath prep table. Instructed to detail clean, receive additional pest control service, and maintain facility free of pests. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed no chemical test kit for determing sanitizer concentration on site at time of inspection. Instructed to maintain chemical test kit on site at all times. Priority foundation violation 7-38-005. Citation issued.
