15. Food separated and protected
- comments: source of contamination. We observed leaking water from condenser unit onto raw meat(4 pounds of breast chicken,and 15 pounds of pork chops) stored inside dispaly serving cooler in the butcher's area. Food held in unsanitary condition was discarded and denatured by the manager pounds 19,value 86. Priority violation:7-38-005,citation issued
16. Food-contact surfaces: cleaned & sanitized
- comments: beginning of black substance at interior panel of ice bin of the ice machine. Instructed to clean, sanitize and maintain unit.
38. Insects, rodents, & animals not present
- comments: outer opening not protected. Hole at bottom of the roll up door at rear delivery area. Seal and maintain to minimize pest entry

- comments: leaking condenser unit inside the meat display cooler in butcher's area. Repair issue.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: temperature measuring device/sensor for the sanitizing cycle at high temp dish machine not indicating adequately the final temperature of warewashing machine. Presently showed final temp of 180f to 200f, however sanitarian an irreversible thermometer showed temperature of 99f to 132. 4f,after several attemps. Must repair measuring device/sensor at warewashing unit. Priority foundation violation: 7-38-005,citation issued -

- comments: accumulated grease and food on utility connections and equipment surfaces behind cooking line/deep fryer. Must clean and maintain. Observed discharge drain hoses attached to condenser unit inside the meat display in butcher's area with a build-up of black slime substance. Clean and maintain.
51. Plumbing installed; proper backflow devices
- comments: leaking faucet at three compartment sink at bakery area,repair issues.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: all black small receptacle containers inside the stall in women's washroom of staff and customer's must have a lids.
55. Physical facilities installed, maintained & clean
- comments: observed expanding spray foam used as a sealant for openings around ceiling pipes behind stove and above the high temp dishmachine. Must remove. Surface must be smooth and cleanable.
60. Previous core violation corrected
- comments: previous core violations not corrected from 3-16-2020,report#2365816 #55) 6-501. 114 noted clutter at the display section of the bakery prep area. Instructed to clean, organize and/or remove all unnecessary articles to prevent hiding places for pest. #55) 6-201. 11 instructed to clean and maintain floors at the following areas:. . . Floors along wall base and underneath cooking equipment. . . . Floors at the bakery prep area. . . . Floor drains at the bakery area in front of the proffers. . . . Floors underneath the hot holding chicken display unit with grease on the floor and shelves. Must also clean and maintain dirty and dusty ceiling walls and fans at the bakery and where ever needed. Priority foundation violation: 7-42-090,no citation issued
