- comments: no parasite destruction letter on site for frozen tuna and other fish served raw for sushi. Must provide. Priority foundation. 7-38-005. Citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: low temperature dish machine sanitizer level 0 ppm. Instructed to use 3 compartment sink and contact maintenance. Sanitizer must read minimum 50ppm. Priority. 7-38-025. Citation issued.

- comments: observed prepackaged rice bowls rice inside prep cooler at temperatures ranging 45. 8f-44. 6f and chicken at 66. 2f. Instructed to discard approximately 10 pounds of food at cost of $60. Priority. 7-38-005. Citation issued.

- comments: no reminder and disclosure on menu pages for sushi and other raw/undercooked appetizers. Must provide on all menus (printed and online). Priority foundation. 7-38-005. No citation issued.
